Getting Started
If you would like to receive accommodations at LSUE, you must submit forms completed by both you and your qualified professional regarding your disability. Provision of reasonable accommodations is determined individually, based on the nature of your disability and the content of documentation submitted. These forms are listed below. Documents from other institutions (IEPs, 504 Plans, letters from teachers, etc.) will not be determining factors regarding decisions for receiving accommodations at LSUE. Already Registered?
To continue receiving accommodations while enrolled at LSUE, you must submit a Request for Accommodations Form to ODS each semester. The form is included on page 3 of the above forms.

Voter Registration
In compliance with the National Voter Registration Act of 1993, ODS offers students the opportunity to register to vote, assistance with completing voter registration applications, and application mailing service to the appropriate registrar of voters office. Students who wish to register to vote may complete the online application through the Secretary of State's website using the link below.
